CDCR 7385: Medical Release of Information
Filling out this form allows you to receive medical updates about your loved one if they become ill. Without it on file, CDCR will not release any health information regardless of whether you are a spouse, parent, or partner.
💡 Reminder: The medical release form must be renewed every 12 months.
📄 Print the form, fill out your section, and mail it to your loved one—they’ll need to sign it and submit it to the prison’s medical department.
CDCR 7421: Advance Directive for Health Care
This form documents an incarcerated person’s medical wishes for emergency and end-of-life care, such as whether they want CPR, breathing machines, or feeding tubes. This produces a medical order that must be followed by all health care providers. It must be filled out with a doctor who explains choices. Once the person makes their choices, the provider signs the form to make it a legal medical order. A copy is kept in their medical file and follows them if they’re transferred or hospitalized.
BPH 1073: Request for Parole Hearing Accomodation
Filling out this form lets your loved one request accommodations- like language interpretation, mental health support, or disability access- for their parole hearing to help ensure meaningful participation.
💡 Reminder: This form should be submitted as soon as a hearing is scheduled to allow time for arrangement
📄 Make sure it's submitted to their counselor and/or ADA coordinator at the prison.
CDCR 1091 & 1021: Release of Other Information
This form allows for CDCR/CCHCS to share information, including but not limited to giving someone permission to view a person's C-file. It expires in 30 days unless indicated otherwise, and the incarcerated person can limit what records are shared.
💡 Reminder: The person authorized by the incarcerated person as their designee must also complete form 1019.
📄 They can submit both forms to the prison’s records department by mail.
CDCR 1824: Request for Accommodations
This form can be used by individuals who are incarcerated to request disability-related accommodations under the Americans with Disabilities Act (ADA). This can include help with mobility, communication, medical needs, or program access. 💡 Pursuant to California Code of Regulations (CCR), Title 15, § 3085.5(c), CDCR must review and respond to the request within 15 working days.
CDCR 2142: Citizen's Complaint
You have the right to file a complaint against a correctional officer for any misconduct. You can also use this form to file a complaint about any matter you’ve attempted to resolve with the institution through regular channels, such as the Ombudsman, counselors, or other staff, but that remains unresolved.
📄 This document can be mailed to the regional Office of Internal Affairs address as listed on the bottom of page 2 of the document
